Big events and other things don't just happen, and sometimes people have dreams, visions or premonitions about them.
Perhaps sometimes there are warnings about looming or impending disasters and dastardly plans.
Sometimes big mistakes happen, like not having enough lifeboats on the Titanic. At least the inaugural sailing should have been coordinated with another transatlantic ship in those icy waters. It could have followed about 1 nautical mile from it, just incase there were any serious problems.

While there was an ongoing concern about the potential of a plane being hijacked to be flown out of the US, apparently, on that day, no one thought that a plane or planes could be hijacked to be used within the country.
Drills or no drills, it was as if someone thought that it wasn't necessary to have fighter jets capable of responding to a captured or hostile aircraft within minutes. Ironically, a firetruck could respond sooner to a typical neighborhood house fire.
